The Incident

On the night of June 10, 2019, Kevin Durant — one of the greatest basketball players of his generation — returned to the court after missing 32 days with a right calf strain. The Warriors were down 3–1 in the NBA Finals against the Toronto Raptors. The stakes could not have been higher.

What Is the Achilles Tendon?

The Achilles tendon is the largest and strongest tendon in the human body. It connects the calf muscles (the gastrocnemius and soleus) to the heel bone (calcaneus), and it is the primary driver of plantarflexion — the motion of pushing your foot downward. In sport, this translates to running, jumping, cutting, and accelerating.

The procedure involves reattaching or suturing the torn tendon ends back together, often reinforced with the plantaris tendon or a graft if the tissue quality is poor. Durant's surgery, performed by Dr. Martin O'Malley at HSS, was described as uncomplicated — a positive prognostic sign.

Post-operatively, the leg is placed in a cast or boot to protect the repair during the initial healing phase.

Rehabilitation: Phase by Phase

Early Mobilization

The boot is gradually weaned. Active range-of-motion exercises begin — ankle pumps, alphabet tracing. Pool walking and hydrotherapy allow load with buoyancy assistance. The tendon is still maturing; collagen cross-linking is occurring. Strength exercises begin for the hips and core.

Phase 3 · Months 3–6

Strength Restoration

Single-leg calf raises become the central exercise — they are the gold standard for tendon loading. A deficit in calf strength of >30% compared to the uninjured side is common at this stage. The athlete also begins progressive plyometric exposure: double-leg jumping before single-leg. On-court/field movement in straight lines begins late in this phase.

Phase 4 · Months 6–12

Return to Sport

Multi-directional movement, agility drills, sport-specific cuts, and controlled scrimmage exposure. Psychological readiness is frequently underestimated here — fear of re-injury is real and must be addressed. Strength asymmetry targets: less than 10–15% limb-to-limb difference.

Phase 5 · Months 12–18+

Performance Restoration

Return to competition does not mean return to pre-injury performance. Research suggests that true neuromuscular and tendon adaptation to game-level load takes up to 24 months. This phase involves continued high-load tendon strengthening and accumulated game minutes.
